Aerococcus urinae is a rare organism infrequently isolated from cultures. Mostly known to cause urinary tract infection, it can cause bacteremia leading to severe urosepsis and infective endocarditis. Embolization is frequently reported with Aerococcus urinae endocarditis (AUE); hence, the presentation is highly variable.
The disease caused by Aerococcus has been underestimated, but mass spectrometry could be a simple method for identifying this pathogen. In this study, we report 2 cases of urinary tract infection by Aerococcus sanguinicola. A descriptive clinical-microbiological study was conducted on the presence of A. sanguinicola causing urinary tract infections.

Balvinder Mohan1, Kamran Zaman2, Naveen Anand3, Neelam Taneja4 1 Associate Background: Aerococcus species may be isolated from urine, blood or other For bacteriuria, cases were stratified into symptomatic urinary tract infection (UTI) if it was from a catheterized urine specimen wouldn't most closely related to Aerococcus viridans.
have been isolated from the air, soil, water, and human infections (Footnote 1, Footnote 5, Footnote 9, Footnote 11). A. viridans has been isolated from blood, urine, synovial fluid, and CSF (Footnote 5). A. urinae and A. sanguinicola have been isolated from urinary tract samples and blood (Footnote 1). Almost 80 % of patients with aerococcal bacteruria had symptoms of a urinary tract infection. Several antibiotics, including nitrofurantoin and amoxicillin, could serve as treatment options. Resistance against ciprofloxacin was recorded, especially in A. sanguinicolaisolates.

Most of the patients (11 of 15) had at least one predisposing comorbidity related to the urinary tract system (8 with recurrent urinary tract infection, 7 with urinary incontinence, 3 with an indwelling catheter, 2 with renal stones, and 1 each with benign prostatic hyperplasia and a recent cystoscopy).

A. sanguinicola is similar to A. urinae in clinical implication, as both were first described as causes of urinary tract infections ( 6 , 24 ) and then later found to be associated with bacteremia, sepsis, and endocarditis ( 4 , 12 ). Aerococcus urinae can, in older males with underlying urinary tract conditions, cause invasive infections such as urosepsis or infective endocarditis. The prognosis of invasive aerococcal infections appears to be relatively favourable despite the old age of patients and their many comorbidities.
